When you call to make an appointment with our office in Annapolis, MD, you will be given a code and password to go online (Click Here) and fill out your medical history as well as consent forms. It is recommended you change your password to one you can remember. Filling out the consent forms does not commit you to treatment, it is only your acknowledgement that you read the form. Any questions you have about it can be answered by the staff or doctor when you visit for your appointment. Once the information is filled out it will automatically update at our office, minimizing your time filling out information on the day of your appointment. We will call you the day prior to confirm your appointment and give you an estimate of your out of pocket costs based on the insurance information you provided. If you were unable to fill the information in on line prior to your appointment, it is expected that you will arrive 30 minutes prior to your appointment to electronically fill your forms out in office. We will then be able to give you an estimate of your out of pocket costs.

You will need to complete a medical history and we ask that you bring with you a list of all the medications you are taking. (If you like, you may download and complete the forms ahead of time by clicking on the links below.)

The next step is usually an endodontic evaluation and treatment if needed, which is most often completed the same day, depending upon the treatment required. One of our staff members will escort you to the treatment area, check your blood pressure, and obtain the necessary radiographs. Your doctor will take time to review your record, discuss your symptoms, answer any questions you have, and provide a thorough diagnostic evaluation before beginning your treatment. At the end of your visit, we will make sure you understand all that has been accomplished and what is the next step in your dental care (usually following up with your general dentist for a permanent filling and continued treatment).

Please assist us by providing the following information at the time of your appointment:

  • Your referral slip and any x-rays your dentist sent with you

  • A list of medications you are presently taking
• If you have dental insurance, please bring your insurance card

Important: All patients under the age of 18 years of age must be accompanied by a parent or guardian.
